Community Outreach

Community Outreach assists socially isolated residents of the

West End / Highgate Hill area . This program provides assistance to access community, participate in social activities, reduce feelings of loneliness, boredom and isolation, develop friendships and feel welcome in their community.

The types of activities which the client and their visitor may engage in, include but are not limited to-

• Individual social support
• Direct linking to sporting or recreational groups
• Shopping or attending medical appointments
• Introduction and referral to support services
• Group social outings

To be eligible for Community Outreach you must-

• Be living in a boarding house or hostel or marginalized accommodation
• Be a resident of West End or Highgate Hill
• Be socially isolated or require assistance to access community

Priority of service is given to people relative to need or who have a disability.
